Erie HealthReach Waukegan
One of our two continuity clinics is Erie HealthReach in Waukegan. This site is a Federally Qualified Health Center (FQHC) and provides comprehensive primary and preventative care to all patients, regardless of ability to pay, health insurance status, or citizenship status. Here, we serve a predominately Spanish-speaking community. Many of our patients are from low-income households, uninsured or underinsured, or undocumented.
Most of the patients served by Erie were without access to primary care until the clinic opened in 2014. As a result, the disease burden seen by our residents is the highest of any of the Erie Family Health clinics across the Chicagoland area. As a result, our residents quickly become comfortable with complex chronic disease management, behavioral health care, and navigating the healthcare system that threatens to perpetuate these disparities. Ultimately, our goal is to optimize health for all patients both through our care and through advocacy and healthcare system change.

Erie offers an income-based sliding fee payment plan to those who are uninsured and select prescription medications at discounted rate. For those patients who require subspecialist services that are not available within our clinic, we have an established partnership with Northwestern Medicine which allows our patients to see Northwestern subspecialists at NLFH free of charge.
